Service Coordination is supported by the Primary Care Partnership (PCP). Victoria's PCP strategy brings together local health and human service providers who work together within voluntary alliances to improve access to services and provide continuity of care for people in their community. Campaspe PCP is one of 28 in the state who are working closely together with their partner organisations to improve the health of their community. The "Virtual Tour" has been a project of the "Campaspe PCP Early Intervention & Integrated Care Committee", with a Service Coordination Framework focus. The aim of these media clips is so that new staff, service providers and general public are able to learn about, and engage in, our local service system. This project hope to bring services and their providers together to promote "no wrong door' as consumers navigate the health and community system. For more information, contact [email protected]
